Special circumstances may require additional documentation, but the following is a general list of what to bring to a tax appointment: photo identification, Social Security cards for everyone on the tax return, birth dates for everyone on the tax return, wage and earning statement(s) from all employers (Form W-2, W-2G, 1099-R), interest and dividend statements from banks (Forms 1099), other relevant information about income and expenses, total paid for day care and day care provider's Tax ID number, proof of health insurance or Marketplace eligibility for the Affordable Care Act, a copy of last year's Federal and State returns if available, bank routing numbers and account numbers for direct deposit.
